Reservation Requirements:
GRADE 7 STUDENTS
- Photocopy of school ID
- Recent Report Card (at least until 2nd quarter)
- 500 pesos reservation fee
- Good Moral Character
ESC Requirements:
- One (1) Long Folder
- Photo copy of PSA Birth Certificate
- Two (2) copies of 2x2 recent and identical pictures (white background)
- Latest Proof of Income: ITR or COE
- Letter of Intent for ESC Subsidy (reason why in need of ESC subsidy)
GRADE 11 STUDENTS
- 500 pesos reservation fee
- Latest copy of Report Card (certified true copy) with school dry seal
